Well, why do people consider one school better than another? Why is MIT considered a strong engineering/sciences school? It’s not arbitrary, there are real reasons these reputations exist. In The Transfer Book we have a whole 40-odd page section on how to determine where you should transfer to.

On the second question: you can talk generally about the things you’re looking for in a new school (DON’T mention prestige – but focus on the reasons behind the prestige). You can usually talk more specifically about each school in each school’s supplement. Also, the common app lets you create a limited number of different versions of the common part of the application. Which approach you should take depends on what kind of supplemental questions each college asks, etc.
